As a home health nurse or case manager, you will use your skills and expertise to care for patients with a wide range of chronic and complex conditions. You will work closely with your leaders and the other members of the home health team to implement your patient’s care plan. Teaching is key, as you will be responsible for educating the patient and their caregivers about their conditions, safety issues, and factors affecting the road to recovery.
Your work as a hospice nurse will have a profound impact on patients and families facing the end of life. You will use your nursing skills and expertise to care for your patients during a very challenging time. You will also serve as the “hub” of the hospice team, connecting the care providers and services needed to ensure that your patients get all the physical, emotional and spiritual support they need and that their loved ones can confidently care for them when you aren’t there.
As a home health or hospice LPN, you will be an important member of the nursing team. You’ll work closely with RNs to help support our patients and families, providing care that may include symptom management or helping patients who experience a medical crisis.
